Prior to prescribed medication being administered at school the following is required:-

You will be required to upload the Medication Authority Form and supplementary page (if applicable) when completing the form.
This form will be reviewed by the prinicpal and you will receive an email advising of the outcome.
Medication Authority Form
For long term medications, a Medication Authority Form (MAF) must be completed and signed by a medical practitioner outlining all prescribed medication for the student.
We prefer the medical practitioner completes the Arranounbai School MAF Template but we will accept other MAFs if they provide the same information.

If the medication is to be administered via PEG then a supplementary page is required

Supply of medication
Medication is to be supplied in the original packaging with the pharmacy label.
If the medication requires cutting to administer the correct dosage then the medication must be supplied in a blister pack.
The medication must be delivered to the school securely by a responsible adult and handed to a member of school staff (NO medication to be in school bags).
